Just watched this. Somehow missed it during the holiday rush. You are spot on. But lets add one more aspect. What about medical emergencies? In todays world very few people think about what it takes to 'take care' of a patient. Lets talk about a simple case, abdominal pain. After admission to the ER, IVs will be started, ECG monitored, and other vital signs recored for a 'history'. Okay, so far so good as in a PAW we can do that. But, here comes the blood work, X-rays and possibly a CT scan, which determines whether you have a ruptured appendix or some other cause like oh, lets say a abdominal aortic aneurysm. Now lets say its just appendicitis, okay that sounds awesome, but is it? Well, yes, the surgeon is called, a STAT operating suite is held, your blood is typed and crossmatched for JIC, the operating team is waiting for you. And for those that do not know, TEAM is what it is. These are people that have done hundreds or thousands of surgeries together. They have the full force of the hospital behind them with all the equipment you can think of, abdominal trays full of specialized instruments for the surgery, the oxygen sources, more meds then you can think of, the right OR bed/table, warming devices, cooling devices oh, and most important for you, someone who will pas the gas (LOL) so you can sleep comfortably during the surgery. So surgery went well (maybe done open or just with tiny holes), you go to the post op ward and wake up in a hour or two. Pain, well you have a nice little device where you can give yourself the pain meds and if that doesn't work, well you have IV access for that breakthru pain, and have 24/7 nursing care. You may have a NG tube in place and other tubes as well.
The point being, like Hawkeye says in an upside down world, we may not have access to EMS, Hospitals, Helos, PJs, Medics, urgent care, etcs. And no matter who you have for a doc, unless they have access to a hospital and all that goes with it, they can only do so much. Can I explore for that bullet in a shoulder, yup, but do I have access to antibiotics to treat the patient with, since surgery will be done in an austere environment and not a nice sterile OR? What will I use to cauterize a bleeder when I get into the shoulder, a red hot knife tip? How will the patient feel about that when I touch them will that red hot poker and he is not asleep? What happens when he jumps and wiggles from that 800 degree piece of steel that was just pulled out of the fire?
I have spoken to my tribe many times about this very thing and even more then what I just typed (it could go on and on). We may be more prepared in our minds, but in reality we are not prepared. There is only so much I can do. If your tribe does not have a medical person, look for someone with a few years on them. OK, look for anyone you can get, but someone who has worked on patients before a lot of the medical automation/tech stuff will transition better, just my opinion.
Also, Hawkeye eluded to trauma scenarios, but in reality I believe medical issues will cause more loss of life. Things like vascular/cardiovascular disease, BP issues which can lead to kidney problems, Diabetes issues, sanitation diseases, tropical diseases, lime disease, kidney and galbladder/pancreatic stones, liver issues, parasites, strokes for high BP and the list can go on and on....oh, and psych issues when people come off of their psych meds. So many things to think and talk about. If you have not thought about and discussed things, now is the time....something to end on, what will you do with the dead bodies? Have you dug a grave before?

TQ's will be almost pointless to carry, as they will likely only delay the inevitable and prolong your pain and suffering.

A TQ could also buy one enough time to allow one to ensure their family is a safe distance away or able to reach safety. Kinda the whole "let me die in a pile of spent brass" notion But, yes, prolonged pain and suffering if no further care is obtainable would really suck.

No one likes to hear it. Especially micro-rig flannel guy who has 5 TQs on his belt. One for each appendage, like he’s gonna be able to keep his composure long enough to apply TQ number 2.
Come on, cousin! Where you going for your transfusion and trauma treatment with 5 tourniquets shutting off your limbs?
Folks need to understand that if you need that TQ or that pair of chest seals, you’re not getting back in the fight…ever.

No one likes to hear it. Especially micro-rig flannel guy who has 5 TQs on his belt. One for each appendage, like he’s gonna be able to keep his composure long enough to apply TQ number 2. Come on, cousin! Where you going for your transfusion and trauma treatment with 5 tourniquets shutting off your limbs? Folks need to understand that if you need that TQ or that pair of chest seals, you’re not getting back in the fight…ever. Umm listen buddy. With the right flannel..you can conquer all.lol I carry 4.TQs They just aren't for me. My tear off pouch is for guys like PSD to keep my hdls ass alive to stay in the fight for a few more minutes or say good by. Honestly..while we have advances in battle trauma care..in the world we prepare for...we may as well..revert to pre 1900s medical tech. If the actual trauma doesn't kill us..infection will as well being in mobile and cripple for life will be a thing for many. Aka a slow death. I plan to make a peg leg from a 1976 pointac grand prix leaf spring ...and get back to zed smashing. I'll make Ash from evil dead look like a newb.
